Difference between Gout and Osteoarthritis as per presentation-

The spontaneous onset of pain, edema, and inflammation in the metatarsal-phalangeal joint of the great toe (podagra) is highly suggestive of acute crystal-induced arthritis. Other than the great toe, the most common sites of gouty arthritis are the ankle, wrist, and knee. In early gout, only 1 or 2 joints are usually involved. Consider the diagnosis in any patient with acute monarticular arthritis of any peripheral joint except the glenohumeral joint of the shoulder, in which a crystal-induced arthritis is more likely to be due to pseudogout. You should go the details of Colchicinum, Stellaria media (for Podagra in pseudogout and rheumatic arthitis), Urtica urens; these drugs are clear picture of Gout. In gout the character of pain is tearing in type in warm; stinging in cold worse, in the evening and at night and from touch; stubbing the toes hurts exceedingly.

Whereas the progression of osteoarthritis is characteristically slow, occurring over several years or decades. Over this period, the patient can become less and less active and thus more susceptible to morbidities related to decreasing physical activity.
Primary symptom can be deep, aching and sore joint pain exacerbated by extensive use. Also, reduced range of motion and crepitus are frequently present. Stiffness during rest (gelling) may develop, with morning joint stiffness usually lasting for less than 30 minutes.
Initially, pain can be relieved by rest and may respond to simple analgesics. However, joints may become unstable, lancinating, shooting and electric like (lightening) as the osteoarthritis progresses; therefore, the pain may become more prominent (even during rest). Phytolacca decan, Formica rufa are the most indicated remedy for. Worse in cold, damp weather, morning aggravation, better in warmth and dried weather. Mal-alignment with a bony enlargement may occur. Most cases of osteoarthritis do not involve erythema or warmth over the affected joints; however, a bland effusion may be present. Limitation of joint motion or muscle atrophy around a more severely affected joint may occur.
